Default SD-209 Model
Looking for the following:

Control: ATJ2097N it may be ATJ2091N (can tell if it's a 7 or 1)
Plate No.: 20070426 SD-209v91a-03(2097)
Screen No.: dm2007008-p-070205-17hd

It's a SD-209 model, 2 gb, external sd slot.
