Exclamation Player shows Disk error!

I have here a 16 gig Actions mpx player. It looks like ipod nano 4th gen.
Actually it has 1,9 gig, but that's not the problem.
After trying to fix it with MP3 player utilities ver. 3.06 - low lever formatting etc. like in guide - program showed error screen and quit. After that player don't see his memory. When I try to open music directory, it says "Disk error!". Then I tried to do the same with MP3 player utilities ver. 4.00, and everything went smoothly, but player also shows "Disk error".
What could it be?What should I do to get thing back? Maybe renew firmware etc?
Please help, thats not my player. My girlfriend asked me to help her with it..

here's the firmware version:

9.5.54 2008/09/05
QS_AK2025
2008/04/23

ChipGenius shows this:

PnP Device ID: VID = 10D6 PID = 1101
Serial Number: 4512482ADF0FE
Revision: 2.00

Device Type: Standard USB device - USB2.0 High-Speed

Chip Vendor: Actions
Chip Part-Number: ATJ209X/ACU75XX

Product Vendor: (N/A)
Product Model: (N/A)

- In addition: What file system do this player normally have? FAT32 or FAT16?
